Why is I-130 stuck in Intial stage for six years?

My PD is 2004. As I understand it the CVC average processing is 42 months, but my petition is stuck on Intial stage for 6 years. What could be wrong? I know I have to wait from my PD to become valid before a visa is issued, but shouldn't the processing at CVC be completed by now? Shouldn't they send it to NVC where it stays until PD becomes valid?
