    Hey all, I recently got married to my bf of 2 years. I'm currently on an OPT, which expires in May (my visa is valid till jun 2015). We are currently in the process of filing for the AOS (he's an american citizen). We're planning on doing a concurrent filing (I-485 and I-130 together). I had a few questions and it would be great if I could get some info. Thanks in advance!

    1) I've been in the US since 2005. Do I need to get my medical examination done and fill out form I-693 and send it along with the rest of the forms? I know I got all my shots in India, but I don't have any of the immunization records with me. Even if my parents have it in India, it will take them some time to get it to me. Since my OPT expires in May, we needed to get this mailed out asap so that I can get my new EAD before my current one expires. So getting an appointment with a doctor would take time as well. I spoke to an attorney and he said I wouldn't need to fill this form out until I hear back from USCIS - but I'm not entirely sure he's correct.

    2) My marriage certificate shows a hyphenated last name (old last name + my husband's last name), although I haven't changed my anywhere else yet (on the passport etc). The plan is to wait till 2015 to change my name on all my docs. Should I use the hyphenated last name that's in my marriage certificate or my old last name on all the forms we're filling out? (since all my supporting docs still uses my old last name). Right now I filled everything out with the old lastname.

    3) The forms ask for current status on form I-485 in part 1 and also on page 1 of I-130. I read on this forum to use D/S - just wanted to confirm this was correct.

    4) Do I need certified copies of all my docs (passport, I-94, EAD, marriage certificate, birth certificate)? I saw on the USCIS website that certified copies are not necessary. Is this correct? Or should I include certified copies just in case?

    5) Does my husband need to show an affidavit of support? If so, what exactly should we include? We split the rent and utilities on our apartment and we both work fulltime.

    6) I read somewhere my husband would need a proof of citizenship. What could we use for this? He doesn't have a passport. All he has is his birth certificate and driver's license. I've included the birth certificate in the packet that I'm going to send them. Would this be enough?

    7) Form I-765 asks which USCIS office I previously applied for employment authorization with. My answer was Nebraska since that's where I applied before. Form I-130 is asking which USCIS office we'll be filing the AOS with. Which USCIS office should we put down? We live in NY now.

    8) I saw somewhere we needed to fill out form G-1125. I couldn't find this online. Does anyone know what this is?

    9) Do we need to send in pics of us and affidavit from friends? Or is this just needed for the interview?

    I know it's a lot of questions, but any help would be greatly appreciated. Thanks!

    You have so many questions in your message that it would take an hour to do justice to a response to you, so I'll start with the last question. You can download the G-1145 from: http://www.uscis.gov/sites/default/f...orm/g-1145.pdf You refer to it as "G-1125," which is incorrect, as there is no such form.

    I apologize for seeming to scold, but very few visitors to this form have the time to respond at length to the number of questions you pose in a single message.

      Thanks, Ray B. I'd already downloaded G-1145. I'd read about the G-1125 form on a different forum so just wanted to make sure I wasn't missing anything. I'm a little nervous about this whole thing. Appreciate your response!
