    I have applied for green card for my brother under "siblings green card" category. I recently received "request for evidence" letter asking to prove my siblings relationship.

    I plan to send copies of my birth certificate along with my brother's. There is a glitch though. Comparing the 2 birth certificates I see that the first and last name of my Mom&Dad got swapped for my brother. Biologically we are the off springs of my parents, however in the birth certificate the first and last name of parents are swapped (for example, my Dad goes as Bob Smith versus my brother got this as Smith Bob). My Mom is no more.

    Should an affidavit from my Dad stating Bob Smith and Smith Bob represents him and that we are the off springs of him would suffice to work around this glitch ? Can I have a sample of affidavit ? should this be issued by a lawyer ? Should my Mom's death certificate be provided as well ?

    Your situation is almost as challenging as a couple I know who are Pakastani. For some reason,a USCIS staff person decided that the beneficiary was from India and they assigned New Delhi as his interview location

    Anyway, it's hard to provide an easy answer based on your problem description without seeing the birth certificates at issue. I know that for many countries, naming conventions seem to go either way, depending on the clerks who completed the civil documents.
