applying from outside USA vs. from inside USA

the second reason i want to apply from usa is that my wife never lived there and it would be easier to prove domecil from there.
my questions will be the following
-if i went on B visa and i applied from there how long will it take to get my Green Card?
ABOUT SIX MONTHS.
-if i applied from there can i use my real authenticate marriage certificate or do i need to re-marry there to make it official. i mean will they accept it even the marriage happened outside usa.
PROOF OF YOUR MARRIAGE OUTSIDE THE U.S. WILL BE ACCEPTED WHEN YOU SUBMIT YOUR PETITION AND ADJUSTMENT PAPERWORK.

from your experience on this did it every happen that someone was rejected because he was on B visa and did the AOS. i mean is that even an option. i mean i am already married to citizen it is not like i will be going there to get marry or something. because i was reading on other forums about the Visa fraud issue and i don't feel like i am doing that.
