Questions about Humanitarian Reinstatement

Hello everyone

I'm currently preparing to apply for Humanitarian Reinstatement and according to the website - I need to mail it to the office that originally approved my petition. Would this be the address located on the bottom of my I-797 Approval Notice? In this case it would be the California Service Center in Laguna Niguel.

Also do I need to wait for some kind of confirmation that my original visa application has been cancelled due to the death of my petitioner or can I just send my Humanitarian Reinstatement in and they'll sort it out from there? and if I do need confirmation who would I speak to to get it.
