Police Clearance Certificate format/example

My mother has I485 interview last week, during interview the officer asked that her fingerprints were not cleared, so he asked for taking her Oath and asked for more documentation like Police clearance certificates from all addresses she stayed in last 5 years.
The interview went on for more than a hour and he asked many questions which I myself never faced in any US visas or any other situations.
Please let me know if anyone has recent experience on asking for such police clearance certificate and if you have any format to get the letters from India (Maharashtra).
