    Hello Experts,

    My spouse submitted form I-130 and other required forms (based on marriage). I'm the beneficiary. I submitted form I-693 in an envelope sealed by civil surgeon (along with all other forms).

    After two weeks of submitting the form we got receipt numbers in email today. While reviewing all the forms today, I was shocked to see my copy of form I-693. The civil surgeon forgot to fill his Name, Address, and contact information on Page 3 of the latest edition of the form. Page 4 has his stamp and signatures though.

    Question: Should I wait for an RFE from Immigration or just go back to civil surgeon and get page 3 (only) completed and sealed in envelope and submit along with an explanation letter to USCIS?

    Please advise!

    I think you should go back there and let you surgeon know what he'she forgot to do. The USCIS officials are VERY "uptight" when it comes to not filling out every little thing. But do not worry too much, your spouse will not get denied for that, you just have to go back and ask the civil surgeon to fill out that one specific page.
