    My mother goes by her middle name in her Passport and on my birth certificate.

    ie. Mary Jane Smith on her birth certificate

    Goes by Jane Smith in Passport and Birth Certificate.

    In my marriage certificate I have her as "Mary Jane Smith'
    Which names should I use to fill out forms for my AOS and my Green Card by marriage to an American citizen?
    - Jane Smith
    - Mary Jane Smith

    I could provide proof with a copy of her passport(Jane Smith) and her birth certificate(Mary Jane Smith), however I wonder will it even be an issue.

    In order to issue any visa, USCIS normally follow the name on a person's passport and birth certificate. So in my opinion use Jane Smith.

    Since case is related to YOUR green card, NOT your mother's so I believe difference in your mother's name will not be an issue. Good Luck.
