    Hello,

    My spouse (Canadian) was admitted to the US on a B1 visa--she received stamp in her passport upon entry indicating B1 visa, purpose, and validity period written by hand by the immigration officer, however no visa number was issued as far as we can tell. For the Nonimmigrant Visa Number in part 3, should she write "None" or "N/A", or does she have a visa number that she's not aware of?

    Secondly, we are unsure of whether to include her middle name on the I-485, or on any of the other forms for that matter. Though her first and last names are the same on all documents and ID (and have never changed, even after marriage) her middle name which is written on her birth certificate is NOT written on her passport, and is also not included on our marriage certificate. We are thinking it should be included on all forms since it is her full legal name, however will there be issues if her middle name is not included on her passport and on the marriage certificate?

      She didn't have a visa -- Canadians don't need a visa to travel to the US for most nonimmigrant categories. Just put something like "(N/A - Canadian visitor)"
