I was a heavy smoker for 30 years more or less... no issues whatsoever. I quit about 3 years ago.
I-693 is blood test‚ urine test and skin test for TB plus vaccination records. Smoking never came up

No, a history of smoking is not a problem for the purposes of Form I-693. The civil surgeon does not need to report it as a Class A or Class B condition unless it is linked to a specific medical issue that affects the person's immigration eligibility. According to the USCIS Technical Instructions (Digital Box 12000), smoking history alone does not require special mention unless it is part of a related medical diagnosis.
