Mismatching names in marriage cert and birth certificate

Hello,

I am a US Citizen sponsoring the GC for my wife (Indian citizen) via concurrent filing of i130 and i485. We have 2 name related issues in her documents:

1. Marriage certificate the middle name and surname are interchanged - so if passport name is 'Jane Mary Doe', the marriage certificate says 'Jane Doe Mary'.
2. In the birth certificate only the first name is mentioned i.e only 'Jane' is mentioned from the above examples.

What do we need to do for this?

The affidavit is for name change after marriage. What should we say if it is a name mismatch? Also, what steps do we need to take for affidavit - notarization?
