It should have your original maiden name. Your married name only came into the picture post-marriage, and the marriage certificate documents that.

hello, the marriage certificate for my parents that we received a few months back has my mothers married name. it has no mention of her maiden name. will this be a problem. the municipal office put in all the information in their system after we provided all the documents to them. should the certificate have the maiden name. and then do we have to provide a affidavit supporting the name change after marriage.

i do have a very old copy of an affidavit from my mother stating/declaring her name change. so if i submit the current marriage certificate shud i submit that affidavit also. am so confused. i thought since the govt. officials are putting the info, thats how it shud be. we just made sure to check the spellings, address, dates etc. any advice. ty.

          Hello Guys,

I have a concern, and I am lost a little bit. If you anyone can tell me what to do. I am green card holder, and I filed the petition for my spouse, and it has been approved recently. the Next step is NVC. My spouse when he was born he had four names on his birth certificate (one first name, one last name, and two middle names). Later on when he grew up he never used one of his middle name, so all the documents such as our marriage certificate, his passport, etc. shows only three names (one first name, one last name, and one of the middle names). When I filed the petition I-130 I used the 3 names as reflected in the marriage certificate and passport etc. I didn't include the second middle name in the I-130 and I-130A. My concern is since his birth certificate shows four names, what can I do when I will fill out the DS-260, and when is going to be the time to submit document with the birth certificate. Can you guide me please!!! Thanks in advance.
