medical examination ect

I'm here on an E2 visa, I recently got married so my husband via my company's lawyer will file for my greencard. Beside all the required documents, it says medical examination, I'm scared of dr, so what can I expect, the lawyer said they are making sure I received all shots when I was a kid???

Also I thought I would only need the I 485 and the I 130, but they also want to file a I 765, not sure why I need it, I already have a social security card and been working in the US for 4 years (E2)?

Found my shot records, but they are not in English, also my name is written with a pen, but it's an official document, I received most shots when I was a kid, are they still valid?
