Concurrent Filing i130, i765, i131 & 1765

Hey Guys

I am working through the paperwork for concurrent filing for Green Card through my wife who is a US Citizen. I am on H1B Visa on valid extension right now.
the original i94 date expired in 2016 but the h1b extension is valid until 2021. also my old passport that I used to enter US has expired but I renewed it in due time before old one expired.

My Question is about filing the i94 sections on i130, i765 and i485

how should I answer following on i130
a) date authorized stay expired or will expire as shown in i94 or i95 --- should it be the date on original i94 which is expired or the date from recent recent i797a h1b extension??
b) Passport Number --- should I put in the old passport number that was used to enter US or new passport number?

how should I answer following on i130
a) Passport number of your most recently issued Passport --- I know it states most recent passport but the instructions are confusing.

how should I answer following on i485
a) Expiration date of authorized stay shown in Form i94 --- should it be the date on original i94 which is expired or the date from recent recent i797a h1b extension ?

When you were approved for Extension of Status and got an I-797A approval notice, a part of that page should be your new I-94, and that's your only valid I-94 from then on. You should put that latest I-94, not any previous ones, when asked about your I-94 and date authorized stay expires. For passport, if it doesn't say the one used at entry, then the most recent one should be fine.
