Need advice on DS-230

i need to fill out DS-230 for my relative, (i'm a USC) he lives in Ecuador but visited US about 15 yrs ago with a passport using a **** name, he even obtained a **** SS card and after a short period of time left USA (about 13 yrs ago) and never returned again, my question is what do i put on line 25 under visits to US? should i put that he visited on certain dates and what about Visa status? Not known?. I was thinking not to put any entries but i don't want to risk it just in case if they find any type of records under his name. what do i do?
