Urgent! - Name change affidavit for Mother's I-130

My birth certificate has my mother's maiden name but she has used her married name in passport, i-94 etc.

1)I have a joint affidavit provided by my father and mother that she also uses her married name by virtue of her marriage. Will this suffice or should other affidavits be provided (by close relatives etc.)?
2)Their last names are abbreviated in most places, should any affidavit be provided to clarify that?
