DS-230 for minor child

I am the registered sponsor and agent for my brothers immigrant visa and trying to help him to fillout his DS-230

Please help with the following questions:

1. One DS-230 application for the entire family or one form for each person?

2. According to the instructions, "Each family member that is eligible to
travel to the United States with you under this visa classification is
required to complete the DS230 Part I"
Applicant's daughter is 3 years old. Does the child need to file a
separate DS-230? Who signs fo rthe child?

3. If not, how does the child gets included in the application process?
