Need Sample Affidavit for father-child relationship for sponsoring parents GC

Hi,

I am looking for sample affidavit which I can get from my relatives as a proof of my relationship with my parents. I do not have my birth certificate which I could have used for this purpose.

Please let me know if you have used this affidavit in the past to sponsor GC for parents and what is the exact text in the affidavit.
