Mother Affidative for Birth Certificate

I asked my parents to get my Non- Availabilty of Birth Records certificate, and then to make to affidavits in the name of my father and mother .(for the Green Card purpose) In the affidavit for my mother ..they forget to write her Date and Year of Birth, though space has been left for that.

Is it o.k. if they send me the affidavit without her DOB, or they have to make a new affidavit with her DOB on it.

Please do respond to this..it is urgent.
