HELP!Required document for Form I-765 (C9)

Can anyone verify that following document is enough for Form I-765 C9 categories:

1. Form I-94- Arrival of Departure Card ( it said C9 need not submit this) I have it should i included it even i m under C9 category?
2. 2 Last OPT EAD Card - I've done my bachelor and Master degree here.
3. 2 2x2 Photograph

Questions: Should i include my passport information page with VISA? I m on H1b visa now.
Do i need to include all my I20 and previous F1 visa?

Please help. When i read from the form instruction it just need the above 3 items. i just scare i m missing something.
