What other forms to include with I-130

I'm single and 20 year old and I want to file an I-130 because my mom just received her citizenship. My tourist visa has long expired since I came in the 2000.

Do I need to file the I-130 with the affidavit from my mom? or do I need to fill out the i-485 change of status too?
