What happens is the beneficiary's last name (as it is submitted with USCIS) is not mentioned? My spouse's birth certificate, they refuse to put her last name. I believe they can give only with first and middle name as it appears on school certificate.

Get what they are ready to give. If the last name mentioned is something obvious like the parents' last name on the birth certificate, it is good. Also add additional affidavits from the parents, relatives along with the birth certificate.
