Did not recieve package containing DS-230

Hi,

I filed for my mother for family based green card.

After Filing I-864, we got letter form NV that they are missng some document which is DS-230, which they already send. But I did not get that package. Later when I talke to couple of my friends who are going through the same process, said that NVC is no longer sending the form, is that correct?

Also what are the document they need along with the DS-230? and do we need to fill Part I & II both and send to NVC?

Download DS-230 from NVC website

Pratik,

You can download the DS 230 Part I and II both from National Visa Center website. Yes, you have to fill out both Part I and Part II at the same time and send it to NVC along with all the supporting documents asked for in the instruction packet (also found on NVC website). This includes the Police Clearance Certificate or PCC, birth certificate of your Mom, Marriage Certificate, Biographic data pages (copies) of her passport etc..

Sign Part I only - Part II has to be signed later in front of the immigration officer.
