I-130: Affidavit to set forth familial relationship

Hi, I am a US Citizen and I have applied I-130 for my mother. I received "Notice of Action" saying that I have to "submit an affidavit that set forth the family relationship between the petitioner and beneficiary"
Can somebody advice on the language of such affidavite?
And I am hoping getting notarized is good enough
Please advice as I have a very short period to respond back

Thanks for taking time and responding
I did submit my birth certificate. My birth certificate was in English but clearly did not say about the relation.
I later submitted a notarized affidavit from my maternal grandfather but I think that was not sufficient either.
That is why I am thinking I need to formulate langugae such that it is clear enough for this communication.
Please advice if you think so too.

I -864 Affidavit of support.

is a form which you have to signt and send.
Is agrement betwnw you and USA which you are going to be sponzor (Financial) for you relative.
