GC: Family Based - Spouse

Hi,

I am citizen and filing GC for my spouse residing in US and is on H-1B.

Here are the forms i am sending:
I-130
I-485
I-864
G-325A
I-765

Here are my questions:

1. Am i missing any other form?

2. Am i sending all the forms together ?

3. After reading thru all of them..it looks like i need to send them Chicago, IL...and not NE..we reside in MO. Please clarify.

4. Do i need to fill G-325A for me and my spouse or just one is good ?

5. What type of medical report do they need?

6. Lastly, is there any org or lawyer from this site who i can contact to go thru the list on the phone..just to crosscheck.
