Petition to Remove Conditions on Green Card

Hello,

I am in the process of filing the paperwork for the petition to remove conditions and I was wondering what affidavit form is needed. The instructions say we need at least two sworn affidavits from people who have known us since the initial approval of my conditional green card. I am wondering what affidavit form is needed and where I would get one.
