help on immigrationf forms my for my husband

Hi there,
I am a U.S citizen applying for my husband's green card who is on H1-B.
I have two questions, in my passport I don;t ahve any middle name, so in forms do I leave my middle name empty or is it okay to put my middle intial?

Second question, me & my husband don't leave togather..we are trying to move to one plac.e but because of economy it's hard to find job.(we have lived togather for 8 months before we gor married)
But we have provide follwing docs to proove that we are married:
1. marraige certificate
2. marriage invitation card
3. Joint bank account
4. Statements showing my husbad beneficiary for my Life insurance
5. Statements showing that I am beneficiary for my hubby's Life insurance
6. tatements showing that I am deopendent for my hubby's Health insurance
7. Affidavit of sworn by people who knows our relationship
8. Our wedding invitations' guestbook printout..where ppl commented and wished us.
9. Itinaries of our trip thruout last year , when we visited each other.

Is this enough? Or do I need to show anything else?
And is there any certain order I should submit my all forms and supporting docs? (I ma sending I-130, G325a, I-864, I-485, I-765)
