Family based green card

Hello All,

My wife is filing for a family based greencard. We are filing the following forms

I130
I485
I864
G325A(for both).

Do I also need to include I 765 form along with the application? Is there any other form that I am missing?

I am on H1 Visa already. Do i need to send the I765 application also?
