Help with form I-765

Hello everyone
I'm an international student, currently on OPT, in the process of applying for my green card, based on marriage to a U.S citizen. My OPT expires at the end of June. These are my questions:
1. Based on the form instructions, my category is (c) (9) "Adjustment of Status Applicant." What should I check in the beginning of the form:
a) Permission to accept employment
b) Replacement (I know it's not this one)
c) Renewal of my permission to accept employment

2. When it asks to which USCIS office I had previously applied for my EAD, and the date, which date is it? Is it the beginning of my OPT? or is it the date when I sent my application?

This is new category so choose:
a)

And the date should be "start date" on the actual card you have now. Send copy of the card front and back along with I-765.
