Approximately 5 years, give or take a few. Monitor the monthly visa bulletin at http://www.immihelp.com/visa-bulletin/ to know if your PD is drawing near.

I am in I-130 F3 category, my PD is NOV/08/2004, approved at DEC/23/2009, in NVC since DEC/31/2009.
How long until I/L, medical...etc. (we just started our 9th years of waiting and 4th since approval).

It's an estimate based on the assumption that VB will move 1 month per month. No one knows for sure how many days/months it will move so most people use this assumption.

Reposting:-I posted this in this forum about Year Ago. (with statistics from USCIS)

846,000 Applicants are waiting for Visa at NVC as of Nov, 2011 under F3 visa Category.
831,000 Applicants are waiting for Visa as of Nov, 2012 Under F3.
1.8% Decrease in the numbers is good news. But it does not make significant change in the projected Wait time.

Only ~24,000 Visa' s are issued each year. That means 25+ years of wait time on average for a person
who is last one on the queue.

15-20+ years wait time for low visa demand countries.
25-30+ years wait time for High visa demand countries such as India, Mexico, Vietnam, philliphines.

So don't expect a big Jump in F3 Category. It will be moving very slowly and painfully.
Surely some visa applicant would become ineligible during this wait period and
Some F1 petitions would be converted to F3. So wait time may or may not shrink. Overall expect long wait time.

This estimate is when your PD becomes current, i.e. the visa bulletin shows your PD is BEFORE the date published.
