I-94 has a wrong date

I would like to let you know that my mother reached Detroit on 7/14/2009 and departture date shown as January 13th 2009 instead of January 13th 2010. I would like that get corrected. Also if she has to extend her stay by 6 more months, when do we have to write a letter and contact Homeland security. Is it possible for her to get a greencard as her two daughters are here?
