Hello, I am filing AOS/I485 for my parent. It was suggested that I file medical exam report along with the 485. My dad is traveling to Chicago whereas I am in San Jose, CA, right now. Is it ok if I file in California but he does medical in Chicago, IL by USCIS approved doctor and fedex me the sealed envelope? Are there any restrictions on where the medical exam has to happen?

You don't need to file the medical with I-485, that could be done later. They will send an RFE when they need the form.

No, the doctor will give the results in a sealed envelope. They will tell if any immunization is needed or if the TB test is positive, then they need to do a chest X-ray.
This is the process we did a few weeks ago:
- call to ask about the process and price
- visit the doctor with immunization records
- bring from I-693 filled the first part but without signature
- bring ID (passport in our case)
- they will do a blood and urine tests
- they doctor will do the physical
- after 6 days the clinic called us saying the documents are ready
They give us a yellow envelope signed and sealed (as requested by USCIS) for each of us and a white envelope with a copy of the I-693 with everything the doctor wrote in it. Is important not to open the yellow, only USCIS can open it.
Don't know how long your parent will be in Chicago, but you need to plan it. If anything goes well the results are ready in 5-7 days, if anything is needed, it can take more time.
You can file I-485 without I-693 (do the medical when they request it) and if you are filing them together, you must send them not later than 60 days after the doctor signed the form.Nov 2018 - Package sent - EB - Texas Service Center
