sponsor GC for parents

I am US Citizen, planning to sponsor GC for my Mom. Questions:
1. Do I need to submit my Mom's birth certificate too?
2. If yes, on the Non-avaliabilty certificate should we ask to have Mom's madien name or the name after the marriage(which is currently in her passport)?

Another question:
I have a non-availability birth certificate of mine but it has only my Dad's name. Is that ok? I am applying GC only for my Mom as my Dad is not interested.

I too plan to file for GC for my Mom but did not see the requirement for the birth certificate of mother. In the I -130 instruction doc, it states :
6. A mother: Submit a copy of your birth certificate showing your name and your mother's name.

Can you pls tell me where the requirement for the birth certificate of mother is ?
