Need help on G-325A

I am filing green card for my mother.This My father is no more.In G-325A there is a question for the spouse.One is for current spouse which in my mom's case doesnt exist.the other is r former divorced spouses which again is not applicable to my mom.My question is should I state my father's name somewher or not.If so in which column and how to state he is diseased.Thanks in advance
