Birth Certificate of parent

My father was born before 1940 and there was no official record of birth certificate. The state department website says for India people born before 1970 don't need birth cert but need to produce two affidavit. So here are my questions,
- SInce he is the oldest member of the family, who can provide the affidavit? Is it enough for an younger sister to vouch that I know X to be born at this day for all my life??
- is there a template for these affidavits.
- Does school/matriculation record that shows birthdate is acceptable proof. I keep hearing that is being accepted now instead of affidavit.

My father’s case is similar and I submitted 2 affidavits (one from my maternal grandmother (his mother-in-law), and one from a close family friend who is a few years older than my father) in addition to a self affidavit from my father as well as some secondary evidence documents like his Aadhar card, PAN card and driving license, Indian passport, etc.

Matriculation certificate should also be acceptable as secondary evidence. I’d use it in addition to the affidavits.

So far, I have not received any RFEs but my application is quite recent. Hope this helps.

Thanks verbose for sharing your experience. Can you please share a redacted form of the affidavit - both the 3rd party and what your Dad self-attested? Or is there anywhere you can direct me online that shows the general form for this? Does that need to be notarized in India or stamped/sealed by a lawyer there?

Here's a sample affidavit from a third party. The more closer the relationship to the applicant, the better:
The affidavit provider got the affidavits notarized in India and mailed them to us.
For self affidavits (from my parents), I got them notarized at a local UPS store since my parents were already in US on visitor visa (I did Adjustment of Status application for them)

AFFIDAVIT OF BIRTH OF <Applicant Name>

I, <Name of Person providing Affidavit>, son of <Name>, am a citizen of INDIA and currently residing at <Address> do hereby state under penalty of perjury that the following is true and accurate to the best of my knowledge:

1. I am a past colleague and close friend of <Applicant Name>, residing at <Applicant Address>
2. I was born on <Date> in <City, State, Country> and I know of <Applicant Name>’s birth because of my relationship to him.
3. In (Date, Year), I was living in <City, State, Country>.
4. <Applicant> was born at <City, State, Country> on <Date>, to <Applicant’s father’s name> and <Applicant’s mother’s name>
5. I know that <Applicant> was born on this date because of mine and my parents’ close friendship and association with <Applicant> and his parents <Applicant’s father’s name> and <Applicant’s mother’s name> of <City, State, Country>..
6. <Add more details about personal association/relationship as appropriate , e.g. working in the same organization, living in the same community etc.>

I verify that the contents of the above affidavit are true and complete, and nothing has been concealed therein. I am ready to testify should there be any questions about this matter.

I don't remember the Field office but I know we sent in all the application and paperwork together to Chicago Lockbox. I think there is a website that will direct you to correct place to send the application.
