Filing i-130: template/format of affidavit for sibling relationship as add docs

Hi

I'm looking for some help with regard to a format or template for submitting an affidavit for proving a sibling relation as additional documentation for I-130......Can someone provide me the same on here? Me and my sponsor who is a US citizen were born in India and so can the affidavits be from india or should it be from people living in the US?
