Petition for Relative (Application Forms)

Hi,
Please let me know if these are the only forms I need to mail while filing for a greencard for parents.

1. Form I-130 (Petition for Alien Relative)
2. Supporting documents:
Proof of sponsor US Citizenship (Naturalization Certificate)
Birth Certificate (with parent's names)
Marrigage Certificate (Parent's)
3. G-1145 E-Notification (Optional)
4. Application Fee

Do I also need to file I-864 Affidavit of Support at this time?

Am I missing any other documents / forms? Do I need to file / mail all the documents in one envelope?

No.

I-864 is to be filled & sent only after your I-130 is approved and you have been notified to fill & file I-864.
