I had asked part of this question earlier, so my apologies for putting all the information again.
I have applied for green card for my mom. Biometric is done. I need to take her to the medical test. She has taken the Tdap and Pneumococcal vaccines a few years back in India. Those are the only ones needed for her age according to CDC. The doctor in India is asking if there is any specific format or data that needs to be in the certificate that will be given as proof of the vaccination? I said Name, Date of Birth, Date of vaccines, and passport Number. Is that sufficient?
Any help greatly appreciated.
