Medical tests for I 693

My mother is on B1 to USA in PLANO TEXAS. She is aged 76 years . We plan to file for her GC.

There is an I693 form to be filed by a civil surgeon designated by USCIS.
Mother has taken 2 COVID vaccines and booster in India .
Q1) Does she have to take the MMR , TDab , Varicella , and other 7 vaccines ??( She has taken 2 COVID vaccine doses in India. She does not have record of the vaccines she may have taken in childhood.)
Q2) Do you recommend any specific clinic OR Dr in PLANO TEXAS.
Q3). The price of vaccines vary by various Drs we had approached . Does anyone have any suggestion on the same .

My mom took medical test in L.A last month ..she is 74 and doc only told TB and Pneumonia vacs are required.

I just got my i-693 yesterday evening. I'm 28 and from Missouri. Here's a brief intro what I had to do.

1. Scheduled an appointment with USCIS Recognized doctor (my tip for this : See google reviews before you chose the doctor specially look for the comments that other people's comments about how easy was getting done the i-693 process)
2. My doctor told me to get MMR, TDap, Flu, Hep B. And if your mom ever got Chickenpox the Varicella vaccine is not required. But you can still tell the doctor that she got Chickenpox as a kid or something that's all they want to waive that. (Here I was lucky my mom had a certified translated copy of my vaccination records already sent to me)
3. After getting those vaccinations and getting the lab work done(Syphilis and TB test) the doctor will simply give the i-693 sealed packet to your mom.

1. Did your Doctor require any IMMUNIZATION Letter from Doctor in India to waive other vaccinations ?
2. My mother does not have birth certificate . What evidence should I submit ? Do you have sample affidavit ?

1. My mom has copies of immunization proof from India
2. My mom doesn't have BC either. We managed to get a non availability certificate from her village panchayat
