Police Certificate - Consular processing

I am doing consular processing for my Mother. The case is now at NVC. She lived in two different cities (districts) in the same state. So my question is whether this Police certificate that needs to be obtained is on state level, district level or even local than that.

Also she does not have a marriage certificate as it was never registered. She is a widow and I have my father's death certificate. Will death certificate be enough or need some document for marriage certificate as well?

Too many documents to collect...
