  • Filling Green Card for Parents - Birth Certificate

We are filing green card for our parents and filling up I130 form. Since our parents don't have their birth and marriage certificate and none record exists as this happened during undivided India, we need to submit their self affidavit for birth and marriage. I am looking for format of this affidavit, if anyone else was in similar situation and have applied for same.

    Thanks

I did it recently for my wife's birth certificate. Her mother is alive so she filled this affidavit. If parents are not alive a sibling/relative/friend who are elder than applicant can fill this.
It was accepted by the NVC.

    See affidavit format below.
