Form I-130 Question 14 Arrival Departure Record

Arrival Departure Record
The format on the form seems broken at the 4th place - quite unlike the alpha numeric format on the I-94 Card (I-94 Card does not have a space or hyphen at the 4th place).

Any inputs on why this inconsistency? How should one fill in this record?

Just fill the Q14 with the number from your I-94 as best as you can. Do not worry about the box in which you cannot fill anything.
