Affidavit of Support

My wife recently applied for a greencard for her parents. Her father's application has been approved. We have filled out the Choice of Address and Agent form and paid the fees. We now have received the letter of Affidavit of Support.

Question is I am the only income in my house and my wife is a housewife, we have 2 children, and my parents live with us, how do we fill out the Affidavit of Support? Which form must we fill (I-864W, I-864EZ, I-864, or I-864A)? Thank you.
