Need urgent help - PCC from Passport office

Hello All,
As part of the GC petition, there is a requirement to submit Police Clearance Certificate from the passport office. My wife's passport was issued 4 years back from a different place than her current residence. Does she need to get PCC from the place where her passport was issued or from her current place of residence?

She required PCC from all the places she lived since the age of 16. This will required because in form DS-230 in one clause they will asked where she lived since the age of 16 and so it is required and she will also required PCC from Passport office from where her passport is issued. its looks little complicated but its not
