If this is the FIRST TIME the baby will enter the US since being born, you will only need:

    1. passport for the baby
    2. original birth certificate for the baby
    3. optional - letter from US embassy/consulate specifying that your baby doesn't need a visa

    (3) is optional (but may be necessary) because some airlines do not know that if you're an LPR and this is the 1st time the baby will travel to the US, then no visa is needed for the baby. On arrival to POE the CBP officer will take you to a secondary inspection and fill all the required forms, stamping the baby's passport with I-551 stamp. Green card will arrive in the mail later.
