Affidavit in lieu of birth certificate

I am a US citizen and am applying for my mother's green.
I don't have my birth certificate showing my mother's name on it which is a requirement.

It is not available from the hospital or local authority.
I have to provide 2 affidavits to support that she is my mother.

Does someone have a sample affidavit for this that I can use?
