PERM approved, signature page lost, scanned copy being used to file I140 in PP

Hi,
Does anyone know if we can submit I140 in PP with only scanned signature page? The signature page was lost by UPS (never ship with these crooks) when my attorney shipped it to me. She has sent me a scanned copy to print out, sign and send it back to her along with PP check.

My questions:
1. She feels that USCIS might accept the application, since only 1 page is a scanned copy. Is that true?
2. I have read in many cases that USCIS requests DOL for a duplicate PERM. Once DOL send it, USCIS sends the duplicate PERM to attorney to get the signature of applicant.
3. If 2 is true, do I need to wait until USCIS sends me duplicate PERM to upgrade to PP?

I am also in the boat , our attorney is applying in PP with duplicate PERM signed copy , does any one's approved with duplicate signed copy of perm.
