I-485 processing information

Form I-485, page 2 - part 3 (processing information) asks questions like- Nonimmigrant visa#, where visa was issued, date visa was issued etc. Last time my cousin entered US as H1B worker with valid INS approval, however, she didn't have H1B visa stamp because she had Canadian GC.

Now, how to respond to these questions of Form I-485? I would appreciate so much if someone can help with advice or share experience to similar problem.
