I140 Application filling issue - Very Urgent Please help

Hi everyone,

I am Sampath. As in passport My Surname is Potkapally and my given name is Sampath Kumar

In PeRM application My attorney entered my last name as Potkapally and First Name as Sampath, he kept the middle name blank

So "Kumar" word in my Given name section of my passport is no where entered on my PERM

My PERM got approved last month.

I have to file my I140 this week. Please let me know how I need to handle this issue to avoid an RFE in the future.
I140 application form has Surname(LastName), Givenname(FirstName) and Full MiddleName Sections.

Any Suggestions would be sincerely appreciated!!!
