  • I-94 neither surrender nor given new one at the port of entry

First came to the US on J1 visa issued by the US Embassy in
Pakistan and got I-94 card at the San Francisco port of entry. After
expiration of the first J1 visa, went to Canada by road but not given the old
I-94 card. From Canada got a new J1 visa issued by the US consulate at Vancouver, Canada. At the port of entry, the immigration officer stamped my passport and wrote J1, D/S without giving me any new I-94 card. Now I am on H1 visa with the first I-94 number issued at San Francisco on it. Recently my I-140 NIW approved and applying for adjustment of status. The Form
I-485 required date and place of last arrival in the US which was through Vancouver, Canada, but I donn't have I-94 card for that. Can I use the first I-94 # issued at San Francisco which was my
first entry to the United States, but not the last entry. Also is this
normal that at the canadian port of entry they do not give new I-94 card
and just stamped the passport. Your help in this regard will be highly
    appreciated.
